Name Changes: Please click here to download the Name Change Form. Once completed, submit the Name Change Form and supporting documentation (marriage certificate, divorce decree or other court documentation confirming your name change) via email to the Engineering Coordinator at [email protected] and cc the Engineering Board ([email protected]).
